The magazine is known for featuring it’s cover stars in full drag and has been graced by US actor James Franco in the past as well as singer Jared Leto dressed as Katy Perry.
Gaga wears a fur coat in her cover shot, which is open to reveal one of her naked breasts covered only by a scorpion nipple ornament.
The cost exposes the singer’s nether region, displaying her pubic hair, while she holds herself around the waist with one arm and raises the other over her head.
In-keeping with the magazine’s cover star tradition, Lady Gaga sports an androgynous look wearing a Charlie Chaplin-style black bowler hat and painted black eyebrows.
She also rocks a pointy moustache and clown-like makeup.
The picture has caused a storm on the internet after it was published earlier today, with some shocked at the singer’s full exposure of her pubic hair.
One fashion website commented: ‘Interestingly, and something worth noting, is the outright exposure of her genitals.
'It's one thing that naked female genitalia made the cover of a magazine; it's another that said genitalia is, for lack of a better phrase, (relatively) au natural ... having an artist of Gaga's caliber go against the status quo -- especially for a publication like Candy -- toys with our perceptions of what's considered normal.’
The singer is actually one of a two-part cover series for the magazine, the second of which features Marilyn Manson in a dress.
